Vendor Name		(The name as it is registered with the city)

Please enter the federal tax id or social security number which was used to register your business with us.

Tax ID		(SSN for sole-proprietors)

Current Username		7-100 characters (letters and numbers only)
New Password		8 - 25 characters (must contain: number, uppercase)
Confirm Password
